Senior Care Network
A Good Idea
Description
The Senior Care Network is a part of Huntington Hospital’s Community Outreach department. Since its start in 1984, this program has worked to enhance the independence for older adults and adults with disabilities. Program components include a resource center, care coordination, caregiver support, discounts on health products and services, and a geriatric clinic which offers in-dept physical and mental health assessments. All care coordinators are bachelors or masters level professionals. They offer families and older adults in-home and/or phone assistance with identifying needs, developing action plans, and obtaining necessary services. In addition, they provide caregivers with free stress management and community resources consultations.
This program is supported by public and private funds, fees, and contributions. Additional funding is provided by The Huntington Collection, the Huntington Hospital’s resale and thrift shop.

The program’s mission is to maximize the wellness and independence of adults in their community.
Results / Accomplishments
In 2009, Senior Care Network received 4,401 calls for senior resources and 921 seniors and disabled adults received care coordination. In the same year, 1,384 individuals attended the program’s 26 health education lectures. A client satisfaction survey revealed that 99% of respondents would recommend Senior Care Network to family/friends.
