The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

The goal of TeenRehabCenter.org is to give parents the tools they need to be able to talk to their children and teens about drugs and alcohol. By providing these resources, it hopes to prevent more teens and young adults from substance abuse. It also aims to help those who are already struggling by furnishing recovery information that is tailored both for parents and for teens.
Time spent in treatment is forward progress, since the individual is removed from substance use and negative peer and environmental influences. At least half of teens who graduate from rehab will enjoy an extended period of sobriety. How they follow-up after rehab is over can play a massive role in how strong and stable these healthy habits become.
- Train staff on broad spectrum of strategies to improve nutrition and physical activity, and reduce health inequities.
- Develop regional collaborations of public health departments, organizations, coalitions and communities to participate in a comprehensive approach to improve nutrition and physical activity.
- Develop a media advocacy campaign to raise awareness among policy makers and the general public about key issues of nutrition and physical activity, and their link to chronic disease and health inequities.
- Use the regional platform to achieve changes in institutional practices and public policies, and increase support for public health.

The goal of the Bay Area SCORES program is to reduce obesity, increase physical fitness, and manage cardiovascular health-related risks while improving academic performance in children between the ages of 6 and 12 through soccer, creative expression, service learning, “hip hop for health,” and creative writing.

The goal of this program is to improve colorectal cancer screening rates among older adults.
Participants in the intervention group had significantly higher colorectal cancer screening attendance, as well as having more positive attitudes about screening and placing a higher priority on screening.

Reduced 30-day readmission rate from 18.2 to 8.9 percent over the course of 2 years, resulting in estimated savings of more than $17 million through 1,804 avoided readmissions.

Baylor Scott & White started TelePsych because many of its hospitals do not have any psychologists or psychiatrists on staff. The Hillcrest hospital in particular has seen an increase in patients needing acute mental health care. Its location next to the highway and the dearth of other trauma centers means that the hospital receives 1-2 patients per day with acute mental health needs. Hillcrest has licensed medical social workers in the Emergency Department but struggled without an actual psychologist or psychiatrist available. Their goal in implementing TelePsych was to move mental health patients through the continuum of care efficiently and with empathy.
The hospital staff at Baylor Scott & White Hillcrest has been very impressed with the implementation and ease of use of the TelePsych process and system. Patient satisfaction has increased and staff are able to move patients through the next level of care more efficiently.
